How it works
A dutasteride depot where the hair loss actually starts
Androgenetic hair loss has one principal cause: dihydrotestosterone, or DHT. Dutasteride mesotherapy targets exactly that, with microinjections directly into the scalp.
DHT is produced when the enzyme 5-alpha-reductase converts testosterone. In genetically sensitive follicles, DHT makes each new hair grow back thinner, shorter and weaker with every cycle, until it stops coming through at all. Dutasteride blocks both types of 5-alpha-reductase, lowering DHT more than finasteride, which inhibits type 2 only.
The decisive difference from the tablet is the route of delivery. Rather than flooding the whole body, the drug is placed into the scalp through fine microinjections, where it forms a local depot. DHT inhibition is concentrated at the follicles, exactly where it is needed.
The core idea: maximum drug concentration in the scalp, minimum concentration in the rest of the body. Not zero, but a fraction of what reaches the bloodstream with a daily tablet.
Safety
If fear of side effects has held you back
A great many patients sit in our consulting room with the same conflict: they can see their hair going, they know 5-alpha-reductase inhibition is the most effective answer to it, and they have still read what they have read online. Loss of libido. Mood changes. Post-finasteride syndrome. That worry is understandable, and we take it seriously rather than talking it away.
Dutasteride mesotherapy is hair medicine’s answer to precisely that concern. Because the drug is deposited locally, measurable blood levels are many times lower than with oral dosing, and systemic side effects depend on exactly that blood level. Swallowing something daily becomes 20 minutes, four times a year.
What we will say honestly
We do not promise you freedom from side effects. No serious doctor could. Even with local injection a small proportion of the drug reaches the circulation, and dutasteride has a long half-life. The clinical evidence is promising but still young: there are several controlled studies and broad practical experience, but no long-term studies spanning many years.
What we do promise: a counselling conversation in which the benefits, the limits and the risks are all fully on the table. And the freedom to stop the treatment at any time.
Not suitable for women of childbearing age, during pregnancy or breastfeeding, or where you are trying to conceive and intend to donate blood. We go through the details individually.

Who the treatment suits
Dutasteride mesotherapy is a maintenance therapy. It protects what is there and thickens miniaturised hairs. It cannot bring back follicles that are already lost.
- Androgenetic hair loss at an early to moderate stage, in men and women
- Patients who refuse tablets, or have stopped them, out of concern about side effects
- Anyone who cannot reliably keep up a daily dose
- Securing the result after a hair transplant
- In combination with PRP, micronutrients or topical minoxidil
- Women of childbearing age, pregnancy and breastfeeding
- Areas already fully scarred or bald, with no follicles remaining
- Hair loss from another cause, thyroid, iron deficiency, autoimmune
- Known intolerance to 5-alpha-reductase inhibitors
- Acute inflammation or infection of the scalp

How does dutasteride mesotherapy work against hair loss?
Dutasteride is delivered into the scalp through fine microinjections, where it forms a local depot. It blocks both types of 5-alpha-reductase and so lowers DHT at the follicles, the principal cause of androgenetic hair loss. Because the drug is deposited locally, the concentration in the rest of the body stays considerably lower than with a daily tablet.
Does it really cause fewer side effects than tablets?
Systemic side effects depend on the drug level in the blood, and that is many times lower with local injection than with oral dosing, which is the medical reason for the markedly better tolerability we see in practice. We still cannot guarantee freedom from side effects: a small proportion of the drug reaches the circulation here too. Locally, mild redness, tenderness or small pinpoint bleeds can occur after a session, settling within hours.
When will I see the first results?
Reduced shedding is usually the first thing patients notice, often within a few weeks, and commonly around the third treatment. Visibly denser hair comes later: the hair cycle needs time, and the full effect develops over 6 to 12 months. A before-and-after comparison is only meaningful from about six months onwards.
Does the treatment hurt?
Most patients describe a brief pinprick at each injection point. Anaesthetic is generally not needed; we can apply a numbing cream beforehand if you prefer. The whole session takes about 20 minutes.
What is the difference between dutasteride and finasteride?
Finasteride inhibits only type 2 of 5-alpha-reductase; dutasteride inhibits both types, and so lowers DHT more. With mesotherapy there is the added point that this stronger inhibition is concentrated where it is needed, at the scalp rather than throughout the body.
Is dutasteride approved for hair loss?
In Germany dutasteride is approved for benign prostate enlargement; in some Asian countries it is also approved for androgenetic alopecia. Here, its use against hair loss is off-label, medically permissible, but requiring full disclosure. We discuss in detail what that means for you before treatment begins.
Can I combine it with other treatments?
Yes. Combining it with PRP therapy, topical minoxidil or targeted micronutrient therapy works well and often makes sense. We also use it after a hair transplant, to protect both the result and the native hair that was not transplanted.
What happens if I stop the treatment?
Like any DHT-inhibiting therapy, mesotherapy only works for as long as it continues. Once you stop, DHT inhibition falls away and the genetically driven hair loss resumes. This is a maintenance therapy, not a one-off cure.
Is the treatment suitable for women too?
For post-menopausal women it is an option in selected cases. It is not suitable for women of childbearing age, during pregnancy or while breastfeeding. With female hair loss we examine the cause especially carefully first, since iron deficiency, thyroid or hormonal disorders lie behind it more often.
What does dutasteride mesotherapy cost in Munich?
The cost depends on the size of the area treated and the number of sessions. At your consultation you receive a binding cost plan covering both the initial and the maintenance phase. As an aesthetic medical treatment it is generally not covered by statutory health insurance.
